SCHEDULED MAINTENANCE INSPECTION PROGRAM

We propose to furnish Mid-America Air-Conditioning and Heating's Scheduled Maintenance Program on the air conditioning equipment located at:                             

Under this agreement, we will inspect the equipment described in Schedule A.  We will use trained men directly employed or supervised by us.  They will be qualified to inspect your equipment properly.

Under Mid-America Air-Conditioning and Heating's Scheduled Maintenance Program, we will regularly, and systematically inspect, and report on the condition of your equipment on approximately the following schedule:

Cooling Start-Up --                                       April

Other Cooling Inspections --                    June, August,

                     Heating Start-Up --                                         October

                   Other Heating Inspections --                   December, February

                     Filter Service --

We will make repairs, or perform other additional work you may authorize.

  At the time of regular inspections, we will do the following as necessary, and applicable, in our best judgment, to keep your system operating properly:

X_ Check operation, and condition of major components.

X_ Adjust major components including motors, starters, and drives.

X_ Lubricate all moving parts, add compressor oil as needed, drain, flush, and refill evaporative condensers, cooling towers during cooling season, and drain water at time of winter shutdown inspection.

X_ Perform air filter service, see Schedule A.

X_ Check for refrigerant leaks.

X_ Purge air from refrigerant system.

X_ Inspect air dampers, and damper motors.

Mid-America Air-Conditioning and Heating' s Scheduled Maintenance Program will also be furnished for these items related to equipment in Schedule A.

  X_ Interconnecting refrigerant, condensing water piping.

            X_ Insulation on the equipment, and refrigerant piping.

            X_ Capacity, temperature, and safety devices which control the equipment.

            X_ Steam traps, and manual service values at the equipment.

            X_ Electric wiring from Unit disconnect.

            X_ Cleaning water-cooled, air-cooled, or evaporative condensers, evaporator,
                    or air unit coils

All work under this agreement will be performed during our regular working hours, unless otherwise specified. Monday through Saturday from 8:30 a.m. to 5:00 p.m.

These items are not included in the agreement.

            X_  Piping other than refrigerant, and condenser water piping.

            X_  Air Ducts.

            X_  Water treatment.

            X_  Emergency service.

In the event that additions, alteration, repairs, or adjustments are made to the system by others, this agreement is terminable at our option, upon five days written notice.It is  agreed that Mid-America Air-Conditioning and Heating shall not be liable for injuries to persons, or damage to property resulting from causes reasonably beyond our control, or liable for any delay or default in performing hereunder, if such delay or default is caused by any condition reasonably beyond Mid-America Air-Conditioning and Heating' s control, including, but not limited to, government restrictions, continuing domestic or international problems such as wars, strikes, fires, work stoppages, floods, embargoes, or unavailability of materials.  Mid-America further shall not be liable for prospective profits, or special, indirect, or consequential damages.

It is agreed that, in order to permit us to perform our obligations under this agreement, we shall be free to start and stop the equipment.  It is also agreed that you will provide reasonable means of access to the equipment.

Mid-America Air-Conditioning and Heating's Scheduled Maintenance Program will be furnished by us for the sum of:  One xxxxxxx xxx xxxxxxxx xxxxx Dollars ($ x,xxx.xx) Per Yearpayable on invoice, after each inspection/filter change.

Mid-America Air-Conditioning and Heating' s Scheduled Maintenance Program shall begin on the date of acceptance or agreed upon date for an initial period of one year, and continue in force indefinitely with automatic yearly renewals at an agreed upon increase of 5% per year unless re-negotiated or canceled as per the terms of the agreement.

Mid-America Air-Conditioning and Heating may terminate this agreement by five (5) days written notice any time during the term hereto on the non-payment of any sum of money due and payable.  This agreement may be terminated by either party by giving written notice at least thirty (30) days prior to the beginning of any contract year.  Neither party shall be liable to the other party for any reason because of the termination under the terms of this paragraph.

  This Proposal contains the entire agreement.

  This agreement is subject to approval of an authorized representative of Mid-America Air-Conditioning and Heating.

We are pleased to offer you our comprehensive Scheduled Maintenance service.

 

The Scheduled Maintenance inspection program includes cleaning and inspection of the following critical system components:

 

Condenser and Evaporator Coils

Fan Motors

Electrical Components

Safety Controls

Refrigerant Levels

Condensate Drain

Blower Belt

Expansion Valve

Unit Wiring

Terminal Screws

 

We also offer a heating version of the program that is recommended in the fall. This service includes:

 

Inspection of Burners and Flame Quality

Ignition Systems

Safety Controls

Electric Heat Operation

Proper Refrigerant Charge (Heat Pumps)

Thermostat Cleaning

Cleaning and Adjustment of Pilot Assembly

Leak Checking

Fuel Pressure

Total Performance Check

 

In addition, an air filter check and replacement (if needed) are included with both of these services.

 

After we complete the inspection, we provide a written report on all components, as well as recommendations for any further service needed.

 

Your special price for either our summer or winter service is $69.00 per unit. If you sign up for both services and pay in advance (one year) your cost will only be $100.00 per unit!
